What Is a Fungal Nail Infection?
A fungal nail infection develops when fungal organisms infect (over populate) the skin beneath the toenail.

Not every thickened or discoloured toenail is caused by fungus. Nail damage, footwear pressure and other nail conditions can produce similar changes, so making an accurate assessment is important before beginning treatment.
How Does Clear-A-Nail Work?
Clear-A-Nail uses Controlled Micro-Penetration (CMP) technology to create tiny openings in the affected nail.
The specialist, computer-controlled device uses a fine sterile needle to create perforations through the nail, helping appropriate topical antifungal treatments reach the affected areas more effectively.
The procedure is designed to minimise discomfort, although some sensitivity may occasionally be experienced.
The Clear-A-Nail procedure itself does not directly treat the fungal infection. Instead, it creates pathways through the nail to support the delivery of a suitable topical treatment.
Your Clear-A-Nail Treatment Journey
The treatment programme consists of an initial consultation, followed by three treatment and review appointments over approximately 16 weeks.
This structured approach allows sufficient time for treatment, regular monitoring and personalised ongoing care.
Appointment 1
Initial Consultation & Assessment
Your journey begins with a comprehensive General Foot Health Consultation to assess your nails and overall foot health. We will discuss your concerns, examine the affected nails and consider whether Clear-A-Nail is a suitable treatment option.
A fungal nail test may be recommended to help establish whether a fungal infection is present.
If Clear-A-Nail is suitable, we will discuss your treatment plan, explain the expected costs and arrange a dedicated fenestration appointment with sufficient time allocated for the number of affected nails.
Existing MEDI-BLISS clients who have already undergone a General Foot Health Consultation may not require a separate assessment.
Appointment 2
Clear-A-Nail Fenestration & Initial Home-care Pack
Your affected nails are carefully prepared, including reducing excess thickness and removing loose or damaged nail material where appropriate.
The Clear-A-Nail device is then used to create tiny openings in the nail plate to help suitable topical treatments reach the affected areas.
Your initial treatment package includes home-care products, together with personalised guidance on their appropriate use and measures to help reduce the risk of reinfection.
Clinical photographs are taken with your consent to provide a baseline record against which future changes can be assessed.
Appointment 3
Treatment Review
Approximately 8 weeks after your Clear-A-Nail treatment your nails are reassessed to monitor progress and identify any changes in nail condition.
Professional foot care is provided, including nail trimming and reduction, heel smoothing, moisturising foot lotion and protective nail oil.
Where appropriate, additional Lacuna Fenestration may be performed using a fine diamond ball burr and professional foot care drill to create further openings in the affected nail.
Your home-care routine will also be reviewed, and replacement products supplied if required.
Additional clinical photographs help compare progress and identify improvements that may not yet be obvious.
Appointment 4
Final Programme Review & Ongoing Care
Approximately 8 weeks after your treatment review a final scheduled appointment provides an opportunity to assess your response to treatment with a fungal test and review changes in nail condition.
Professional foot care is provided according to your individual needs, with additional Lacuna fenestration where appropriate.
Clinical photographs are compared to help evaluate progress and identify areas of healthier nail growth.
At this stage we will discuss any ongoing home care or additional treatment that may be appropriate.
Although this appointment completes the treatment programme, continued nail care and monitoring may be beneficial while the affected nail grows out.
How Long Does It Take to See Results?
Treating a fungal toenail infection requires patience.
Even when a negative fungal test indicates that no fungal infection was detected in the sample, it doesn't necessarily mean the nail will look completely healthy. A fungal damaged nail can take many months to grow out, even after an infection has cleared. A big toenail can take approximately 12–18 months to grow out completely.
Clear-A-Nail results depending on the extent of infection, individual nail growth, underlying health factors and consistency of home-care.
No treatment can guarantee complete clearance, and fungal nail infections can recur.

A repeat fungal nail test may be recommended at appointment 4 to check for any remaining fungal infection. The results can help determine whether further treatment is necessary or provide reassurance that the infection has cleared.
Frequently Asked Questions
How is Clear-A-Nail different from applying an antifungal product alone?
Clear-A-Nail creates very small channels through the nail plate. These allow the recommended topical antifungal product to reach beneath the nail more directly.
Does Clear-A-Nail treatment hurt?
Treatment is generally well tolerated. The device is designed to detect changes in resistance as it passes through the nail. Your comfort is monitored throughout, and you should report any discomfort immediately.
Will I still need to use an antifungal product at home?
Yes. Creating the channels is only one part of the treatment. Using the supplied antifungal product according to your individual instructions is an essential part of the plan.
How soon will I see a difference?
The existing damaged nail will not immediately become clear. Progress is monitored as new nail grows, so visible improvement takes time and varies between clients.
Will I need follow-up appointments?
Yes. Reviews allow the nails to be checked, progress photographed and further treatment considered where required. Your appointment schedule will be explained following assessment.
Can the infection return after treatment?
Yes. Reinfection can occur, and complete clearance cannot be guaranteed. Ongoing foot hygiene, footwear care and addressing any fungal skin infection are important parts of your aftercare.
